Tri-Village schools will offer athletic physicals for students
entering grades 7-12 for the 2018-2019 school year from 6:00-8:30 p.m. on May 3rd
and May 14th at Family Health in Greenville. Each athlete must pre-register for a time by
contacting the high school athletic office. Physicals will be completed only for the purpose of participating in
athletics at Tri-Village Schools. Completed physicals will fulfill the student athlete requirements for
summer camps, as well as athletics in the 2018-2019 school year.
Physical forms will be available for pick up in the JH/HS
Office, as well as on the school website after May 1st. Parents must complete the parent and student section
and bring it with them on the day of their athletes physical. Cost will be $10.00. Checks should be made payable to Family
Health.
It is imperative that students do NOT have ANY caffeinated
beverages (soda, energy drinks, etc) on the day of their physical. If there are any abnormal findings, the student
will need to follow up with their primary care physician.
